Changing your last name after marriage in Missouri
Get your name change done. Enjoy being married.
Missouri’s state health department issues a marriage statement, which is different from a certified marriage certificate. For the certificate, contact the Recorder of Deeds in the county where you obtained the license.

With Name & Next
A head start on the paperwork.
Which application applies to you? What documents should you have beside you? Does another update need to happen first?
Name & Next asks about your situation and brings the relevant guidance together, so you can start with more of those questions answered. Have what you’ll need in front of you, including the details worth checking before you submit.

Some services fill selected applications or provide printed materials and mailing supplies. Check what’s included and what you still complete yourself.

Make the timing work for you.
Some decisions are easier to make before you are standing at the counter. In Missouri, your license’s expiration date affects whether your name update is handled as a duplicate or a renewal. If it is within six months, or 184 days, check the renewal requirements when planning the visit.
Name & Next keeps the relevant official instructions beside your preparation, including the title work required for a vehicle registration name change. You can look over the documents and see what to gather while you still have time to find them.

Request your Social Security name update
Open SSA's name-change service to start the request or arrange the appointment it requires.

Original or issuing-agency certified marriage record; an ordinary photocopy or notarized copy is not enough.

- Current identity evidence accepted by SSA. If the marriage was more than two years ago or the marriage document cannot establish identity, SSA may also require identity evidence in the prior name.
- Citizenship or immigration evidence if SSA needs to establish or update that status. Enter identity numbers only with SSA.
The SSA service determines whether your request can be completed online or needs an appointment. SS-5 is a paper companion when that route calls for it.

Is Name & Next right for me?
For adult U.S. citizens living in Missouri and taking their spouse’s current last name unchanged after a marriage recorded in the United States. You can also buy before a planned U.S. wedding and use the same purchase afterward.
Check the full fit before buying
Any state credentials or vehicle records you want to update must already be Missouri records, with vehicle owners staying the same. Having no state credentials or vehicle records is fine. Interstate and ownership transfers, hyphenated or newly created names, divorce, court ordered changes and immigration routes are outside this plan. Read the full eligibility details.
You can prepare before the wedding or while waiting for your certified marriage certificate. Official updates still depend on the recorded marriage and each agency’s requirements.
Is my marriage certificate all I need for the license office?
No. Missouri also asks for documents verifying identity, Social Security number and Missouri residency. A certified marriage certificate establishes the name change, but it does not replace every other requirement. An initial REAL ID can need additional documents. Check the list for your credential and whether your visit will involve a duplicate or a renewal. Review DOR’s license document guidance.
Does changing my registration also mean a new title?
Yes. Missouri says a registration name change requires applying for a title in your new name. For a motor vehicle or trailer, it identifies Form 108, the original title and accepted name change evidence. When ownership stays the same, the title does not need to be assigned to someone else. Confirm the applicable fee with the license office. See the marriage name change instructions and the titling manual.
Should I request a marriage statement or a certificate?
For a certified copy of your Missouri marriage certificate, contact the Recorder of Deeds in the county where you obtained the license. The state health department issues a marriage statement with limited details, which is a different record. If you married outside Missouri, request your certificate from the jurisdiction that recorded the marriage. Compare Missouri’s marriage records.
